Health and Wellness Important Information:
-Receipts must be accompanied by a completed, signed form.
-Receipts must only contain items for reimbursement and be itemized (no debit receipts will be accepted)
-Fund allotment runs From July 1 to June 30
-Forms and receipts can be submitted by email to cupe4165handw@outlook.com, or by interschool mail to Erin Phillips at Gordan Terrence School or drop off at the CUPE Office in Cranbrook. Emailing is the best option as interschool mail can cause delays. Please note for submissions for the August first deadline interschool mail is not an option.
There are 4 opportunities to submit:
August 1, November 1, February 1 and May 1
It is strongly recommended that forms are submitted at least one week in advance of the cutoff date to ensure time for processing.

- Sports Club/ Fitness Membership

Weight Loss Organizations *Excludes the cost of food and supplements
- Personal Trainers*Must be fully certified and registered with a fitness club or recreation center.*Proof of certification will be needed for reimbursement.
- Sport Lessons, Registration Fees, and Passes
- Fitness/ Wellness Apps or Books
- Wellness Experiences/ Classes ie. Art, Music, Language, etc.
- Spa treatments (eg pedicures, manicures, massage (amount not covered by extended benefits))
- Community Centre/ Recreation Centre Activities
- Sports Equipment
- Footwear
- Active Wear
- Home Fitness Equipment
- Smart Watches
- Additional expenses towards medical and dental premiums, deductibles, fees, or other expenditures not covered by the benefits provider i.e. prescription drugs, eyeglasses, sport braces, orthotics, extra visits. *To access this fund, you should use any benefit coverage you have either through your plan or your spouse’s plan prior to submitting a claim for these funds. Proof must be provided for what insurance paid/not paid.

- Beauty Treatments or Cosmetics
- Weapons (Gun/Knife/ Bows etc.)
- Licensing of any kind and hunting tags
- Insurance of any kind
- Camp site rental fees
- Vehicle parts and accessories of any kind
- Swimming pools
- Electronics of any kind (excluding smart watches)
- Appliances and machinery of any kind
- Pet care and supplies
